« Back to Glossary IndexOther Specified Bipolar and Related Disorder (OSBARD): Other specified bipolar and related disorder (OSBARD) — formerly known as bipolar disorder-not otherwise specified (BP-NOS) — is a formal diagnostic category for individuals who experience clinically significant bipolar symptoms that do not meet the strict duration or symptom-count thresholds for bipolar I, II, or cyclothymia. It is the primary home for subthreshold and subsyndromal presentations on the bipolar spectrum. Common examples include short-duration hypomania (lasting only 2–3 days instead of the required 4) or depressive episodes paired with too few manic symptoms (such as having only 2 symptoms, like talkativeness and racing thoughts, instead of the required 3) to qualify for a BD-II diagnosis. This label ensures that people with real-world impairment from mood instability can still receive a formal diagnosis and access support, even if their patterns don’t fit the classic profiles.
