These thoughts are unwanted, distressing, and often go against the person’s values, leading to intense fear, shame, and guilt—but it’s not inherently dangerous. Fears can involve hurting a loved one, a stranger, or yourself, failing to prevent harm from happening, acting violently or impulsively, even when you don’t want to. Harm OCD is common, with research showing that nearly a third of all people report that they’ve had harm-related obsessions.
